You can navigate to the CONTACT US portion of our website and click on the Book Appointment link to set up appointment and fill out forms. You can also call us at (850) 374-3620 to leave a message and someone will get back to you in with 48 hrs. If you choose not to book through our website, please know you will have paper forms to complete and it is recommended you get to your appointment 15 min early, or you can fill them out here by clicking on the 2 links down below:
